Medical Release Form

This tells us of any allergies or special needs your child may have, provides important emergency contact information, and gives us permission to take them to the hospital in an emergency.  Good Stuff!

Media Release Form

We take pictures and shoot videos, but we can’t use your kiddos smiling face without your permission.  Fill this out to give us permission to post their image in the group photo.  Say Cheese.
